Cost/Insurance

Insurance eligibility will be checked on-site to confirm coverage before administering any vaccine. Nonetheless, we advise you to check with your insurance provider about your plan’s specific coverage in advance. While many plans allow you to receive these annual vaccines without any co-pay, anyone not covered for these shots will need to pay out-of-pocket.

Paperwork

Each individual who wants to receive a vaccine will be asked to complete a consent form and bring proof of their insurance coverage. Patients need to have a completed and signed form before they enter the nurse’s suite to get their shot(s). Note that we will not collect completed and signed forms until you are sitting in the nurse’s chair to get your vaccine(s). Paper copies will be available for you to pick up at the front desk at 2nd Street Campus and Cooper Campus. You can also see the form linked below.
